Insurance
We are participating providers for BCBS PPO, PHCS, Cigna PPO, Aetna PPO, First Health, CCN, HFN, The Preferred Plan and Medicare. This means we have agreed to accept discounted rates for our services, and agree not to bill you beyond the “allowed amount.” We ask that you know your deductible (the amount you need to pay out of pocket before benefits begin) and your co-payment so that you may pay this amount at the time of service.
We are happy to bill your insurance carrier directly. However, we must have a current copy of your insurance card(s) and an up-to-date registration form. Insurance companies often request “additional information” from you, the patient, which delays processing. Answering these inquiries in a timely manner will ensure prompt payment. If payment is not received from your insurance company within 60 days, we will bill you directly.
Non-Participating Provider
If we are not a preferred provider in your PPO or HMO Plan, we are still able to take care of you. We are not participating providers for Unicare, United Health Care, all HMO Plans and some other PPOs. You are expected to pay at the time of service. For most plans, there is an “out-of-network” allowance, and we are happy to provide you with the necessary forms to submit to your insurance company.
Medicare
We accept assignment for Medicare patients.
Self Pay
We expect payment at the time of service. We accept cash, personal check, Discover, Visa & MC.
Missed Appointments
Appointments are scheduled for 15 minutes for “sick visits,” 30 minutes for follow-up visits and 60 minutes for complete physicals. If you need to cancel a scheduled appointment, it is imperative that you cancel with at least 24 hours advance notice or you will be charged $50 for a 30 minute appointment or $100 for a 60 minute appointment. Please be aware that you are fully responsible for such charges and insurance companies do not pay for missed appointments.
Billing and Payments

Charges are based on the time involved, the urgency and complexity of the problem, your past medical history and the number of issues discussed.

There will be a $25 fee for forms not filled out at the time of the appointment, depending on the complexity of the form.

Cash, Checks, Discover or MC/Visa are accepted.

There is a $25 fee for returned checks.

If your account has not been paid for more than 90 days and arrangements for payment have not been agreed upon, legal means may be used to secure payment. Legal means may involve a collection agency or small claims court.
Pharmacy Refills
Every attempt will be made by the physician to refill ALL your medications at the time of your scheduled follow-up or physical examination. Medications will not be refilled when the office is closed. Please keep an updated list of your medications with you at all times. Have the pharmacy fax a request to 847.677.7516 for all refill requests. Please allow 24 hour turnaround time.
Urgent Care
If you are ill, Dr. Robledo will make every attempt to see you within 24-48 hours. Slots are kept open in her schedule for this purpose. Please be aware that you may be asked to wait if others are scheduled. We may also have you see our Nurse Practitioner, Patty Klos if Dr. Robledo is unable to see you as soon as you wish.
Contacting the Physician
Non-urgent calls are usually returned within 24 hours. If for some reason you have not heard back from us, please call again, in that, even voice mails can be faulty. If your message is urgent, please discuss this with the secretary or answering service. You may contact Dr. Robledo after hours any time for EMERGENCIES. For immediate situations that cannot wait for a call back, please dial 911. The answering service for emergencies is 815.929.2268.
Test Results
All of your test results will be communicated to you by phone within 7 days of our office receiving the results. We will generally call you sooner if there is a problem with any of your tests. Please do not call before 7 days unless instructed otherwise. If you do not hear from us within 2 weeks, certainly call our office.
